Isn’t the unsubscribe link at the end of each newsletter/Mail sufficient?
Some people will very likely use such a form to spam existing users with unsubscribe emails.

Philipp Stracker: Yes and no... it is just a customers' requirement in our case.
The same way could some people spam users using the subscribe form, as this also triggers new subscribe mails ;)
